Troubleshooting

Source: DICE IOM Manual V12E, Page: 39
Before any repair work, depressurize the system, release regulating valves, and flush all fluids. Wear appropriate PPE.

Module Issues

Possible CauseAction
Insufficient pressure on connector from mushroom nutHand-tighten the mushroom nut (no tools)
O-ring failureReplace the O-ring (30)

Ball Valve Issues

Possible CauseAction
Carrier O-ring failureReplace carrier O-ring (7)
Fluid passes through the valve when handle is in closed position.
Possible CauseAction
Worn ballReplace ball (4)
Worn ball valve seatReplace both seats (6) and seat O-rings (8)
Loose carrierAdjust carrier position per Maintenance procedure
Ball valve does not fully open/close, or requires excessive torque.
Possible CauseAction
Excessive pressure between ball and seatsAdjust carrier position per Maintenance procedure
Excessive torque on stem nutLoosen stem nut and retighten to 30 in-lb
Possible CauseAction
Broken stemReplace stem (3)
Broken stem/handle couplingReplace stem (3) and handle (11)
Broken stem/ball couplingReplace stem (3) and ball (4)
Possible CauseAction
Stem O-ring failureReplace stem O-ring (9)
Stem nut O-ring failureReplace stem nut O-ring (10)

Back Pressure and Pressure Relief Valve Issues

Possible CauseAction
Ruptured diaphragmReplace diaphragm (16)
Possible CauseAction
Ruptured diaphragmReplace diaphragm (16)
Insufficient pressure between diaphragm and blockTighten 4 mounting bolts (18) to 35 in-lb in star pattern
Possible CauseAction
Spring failureReplace spring (17)

Pressure Indicator Issues

Possible CauseAction
Loose NPT connectionTighten pressure gauge NPT connection on isolator
Ruptured isolator diaphragmShip isolator and gauge assembly to supplier for repair
Chemical attack on gauge connectionShip isolator and gauge assembly to supplier for repair
Possible CauseAction
Bleeding screw O-ring failureReplace O-ring
Ruptured isolator diaphragmShip isolator and gauge assembly to supplier for repair
SymptomPossible CauseAction
Reading too highBlocked injection pathCheck for closed valves or blockage downstream
Reading too lowPressure relief valve set too lowReadjust PRV per Startup procedure
Reading zeroSystem not pressurizedVerify pump is running and primed
Erratic readingAir in systemUse bleed valve to purge air

System Issues

Possible CauseAction
Trapped air in systemSlightly open bleed valve (32) while pump cycles until fluid flows
Check valve failure in pumpInspect and replace pump check valves
Suction line air leakCheck suction line connections
Possible CauseAction
Injection valve closedOpen injection valve (1)
Back pressure set too highReadjust BPV to 20-30 psi above process pressure
Pump not runningVerify pump power and operation
Blocked injection lineClear downstream blockage
Possible CauseAction
Pressure relief valve set too lowIncrease PRV set point
Back pressure too high relative to PRVVerify PRV is set higher than BPV
BPV diaphragm failureReplace BPV diaphragm (16)
Possible CauseAction
Calibration column valve closedOpen calibration column valve (2)
Injection valve openClose injection valve (1) during calibration
Separation valve closed (DM only)Open separation valve (4) during calibration
Possible CauseAction
Evacuation valve closedOpen evacuation valve (3)
Calibration column not ventedVent top of calibration column to atmosphere
Blockage in evacuation lineClear evacuation line
